Patient and presentation
A 50-year-old patient presented with several missing posterior teeth that affected eating. The mandibular right posterior teeth had been absent for years, with additional tooth loss in the opposite arch.
The patient reported smoking 10–15 cigarettes daily and light alcohol use; the recorded systemic medical history was otherwise unremarkable.
Clinical assessment
Sites 37, 46 and 47 were diagnosed as missing. Imaging recorded mild vertical resorption at 46 and 47 and moderate resorption at 37, with average oral hygiene.
A three-implant plan was prepared to restore the mandibular posterior spaces.
Surgical treatment
BL.RC4110 implants (Ø4.1 × 10 mm) were placed at sites 37, 46 and 47, each with a BL.HA.RC6020 healing abutment. The sites were classified as Class III bone.
No grafting was used. The implants were managed with a non-submerged protocol, interrupted sutures and good reported primary stability above 35 Ncm.
Restoration and review
One year after placement, the restorations showed close crown margins and no reported peri-implant abnormality. Cement-retained abutments were used for the definitive prostheses.
At five years, imaging continued to show favorable implant position, stable reported first-thread bone levels and no abnormal peri-implant findings.
